PHA650R Ambulatory Care Pharmacy

[0–3, 3 cr.]

This one-month elective APPE is designed to provide Pharm.D. candidates with the opportunity to contribute to the care of patients in hospital-affiliated outpatient clinics and specialty ambulatory settings. Ambulatory care pharmacy focuses on patients with complex, chronic conditions requiring specialized medication management, often involving high-risk or specialty medications. The student-pharmacist will engage in comprehensive medication therapy management, collaborative drug therapy management , and interprofessional collaboration within specialty clinic teams. The student-pharmacist will deliver patient-centered care following the Pharmacist Patient Care Process (PPCP) model, with emphasis on specialty disease state management, transitions of care, and optimization of complex therapeutic regimens.
